Partner Agency Referral Procedures

1. Agencies referring participants to Goodwill for services (Work Experience,
Financial Literacy, Food & Nutrition Classes) will need to complete an electronic
Google Form.

a. All fields with an “ * “ must be filled out to submit form
b. Although not required please answer referral questions as much as
possible. If not enter N/A.

2. Once the referral is received the participant will be contacted within 24-48 hours
to schedule an intake assessment.

3. Once the intake is complete; the referring agency will be contacted to schedule
the store assignment start date.

4. At this point communication with the Store Manager can take place. The Store
Manager is the person responsible for submitting timesheets.

5. When the participant completes the assignment the referring agency must notify
Rachel Loera, Vice President of Mission Services, of the last day on assignment.
